Publishing Clearance
By clicking the 'Submit Form' button below, I hereby grant the University of Kentucky's Painting Technology Consortium (PTC) a nonexclusive, royalty-free, irrevocable, worldwide license to reproduce and distribute to PTW Members and workshop participants copies of my presentation materials in hard copy, in CD/DVD electronically, or on a pass-word restricted website via the Internet. I also authorize PTC members and workshop participants to reproduce and distribute copies of the materials, without charge, to other individuals in their offices or business, provided that they include in the copies any credits, acknowledgments, copyright notice, and other such information contained in the materials. I retain all other rights in and to the materials, including copyright therein.
I hereby verify that the materials are my individual work, unless expressly noted below. If any materials produced by others are included, I represent that I have obtained permission to use the materials, and that appropriate attribution to the source has been given. I shall be solely responsible for the content of my materials.
I acknowledge that it is PTC's policy not to endorse any particular product or company, and my materials will abide by this policy.
